1. Federal Tax Return [PDF]
Upload your most recent filed federal business tax return (Form 1120S, 1065, or 1040 Schedule C) to evaluate historical margins.
2. Profit and Loss Statement (P&L)
Export the Year-to-Date (YTD) P&L from QuickBooks, Xero, or your bookkeeping software.
3. Bank Statements (Last 3 Months)
Please upload statements for your primary operating checking account. (Ensure all pages are included in a single file per month).
4. Payroll Summary Report (YTD)
Download the "Payroll Summary (Year-to-Date)" report from your provider dashboard:
- Gusto: Go to Reports ➔ "Payroll Summary" (Set date range to Year-to-Date).
- ADP: Download the "Payroll History" or "YTD Summary Report".
- QuickBooks: Select Reports ➔ search for "Payroll Summary".
